margin: 0px; padding: 0px; box-sizing: border-box;">阜阳市中医医院康复中心联合放射科于2025年9月3号下午顺利完成了我院首例吞咽造影检查。</p><p style="text-indent: 2.2353em; white-space: normal; margin: 0px; padding: 0px; box-sizing: border-box;">吞咽造影检查（VFSS）是在X线透视下，针对口、咽、喉、食管的吞咽运动所进行的特殊造影。医生通过侧位及正位成像，观察病人吞咽不同黏稠度、不同剂量的造影剂，进一步评估出四个吞咽阶段（包括口腔准备期、口腔期、咽期、食管期）的异常情况。此检查，是目前公认最全面、可靠、有价值的吞咽功能检查方法，吞咽障碍检查和诊断的“金标准”。</p></section><section style="text-align: center; white-space: normal; margin: 0px; padding: 0px; box-sizing: border-box;">常规的吞咽障碍临床评估具有一定的局限性，因不能观察到整个吞咽经过的管道，所以不能提供口腔、咽及喉部结构与功能的某些信息，也就不能知晓吞咽所需时间，不能了解咽的力量、压缩食团的能力或是否吞咽后有残留。看不到喉部，则无法直接观察到患者是否有误吸或误吸是如何发生的。</p><p style="text-indent: 2.2353em; white-space: normal; margin: 0px; padding: 0px; box-sizing: border-box;">对于广大患者来说，吞咽造影检查，也是一项易于接受的检查。首先是费用低廉，目前该检查的花费仅相当于一张普通X片的价钱;其次安全性高，全程无创，所用的造影剂不会被人体吸收，能完全经胃肠道排出体外，第三判断患者安全进食的食物种类。因此，在吞咽障碍治疗工作中，开展吞咽造影检查，有着极为迫切和重要的意义。</p><p style="text-indent: 0em; white-space: normal; margin: 0px; padding: 0px; box-sizing: border-box;"><strong style="box-sizing: border-box;">吞咽造影的适应症</strong></p><p style="text-indent: 2.2353em; white-space: normal; margin: 0px; padding: 0px; box-sizing: border-box;">1.各种原因引起的吞咽功能障碍或反复肺部感染，尤其是隐匿性误吸的疾病。</p><p style="text-indent: 2.2353em; white-space: normal; margin: 0px; padding: 0px; box-sizing: border-box;">2.吞咽困难考虑为食管本身或食管附近病变引起，特别是食管肿瘤、食管痉挛、食管异物等。</p><p style="text-indent: 2.2353em; box-sizing: border-box;"><strong style="box-sizing: border-box;">吞咽造影检查的内容</strong></p><p style="text-indent: 2.2353em; white-space: normal; margin: 0px; padding: 0px; box-sizing: border-box;">该检查是用改良的造影剂(碘克沙醇、舒食素与食物调制成四种不同质地的食物)，在X线机上，在吞咽治疗师的指导下，观察患者进食各种不同质地食物的情况,同时采取一些必要的代偿方式,防止误吸，并用照相机录下整个过程。然后对结果进行分析，评价患者能否从口中进食或需要采取何种营养支持途径;同时指导训练计划，评价训练效果。该方法安全可靠，可取得预期满意的效果。</p></section><section style="text-align: center; white-space: normal; margin: 0px; padding: 0px; box-sizing: border-box;">①口腔期：重点观察口唇的闭合及随意运动、舌的搅拌运动及运送功能、软腭的活动及有无鼻腔内反流、口腔内异常滞留及残留等</p><p style="text-indent: 2.2353em; white-space: normal; margin: 0px; padding: 0px; box-sizing: border-box;">②咽期：重点观察吞咽启动是否延迟、咽缩肌收缩情况、喉上抬幅度、会厌是否翻折、会厌谷及梨状窦是否异常滞留、有无误吸（尤其是并发肺炎的隐性误吸）及渗漏、是否能通过咳嗽将食物清除气道等</p><p style="text-indent: 2.2353em; white-space: normal; margin: 0px; padding: 0px; box-sizing: border-box;">③食管期：重点需要观察食管上括约肌是否开放、开放的程度、食管的蠕动等</p><p style="text-indent: 2.2353em; white-space: normal; margin: 0px; padding: 0px; box-sizing: border-box;">3.辅助临床治疗方案的制定，包括营养或水分摄入的途径、最安全或最有效的饮食等级、是否需要喂食调整或治疗干预</p><p style="text-indent: 2.2353em;
